Do not take another dose to “push through” a poor response or a delayed onset. A clinician should review technique, timing, food, stimulation, other medications, and whether the diagnosis or treatment plan needs to change.

The nitrate warning is non-negotiable

Sildenafil and tadalafil can lower blood pressure. Combining either one with nitrates can cause a dangerous drop. This includes nitroglycerin and other nitrate medicines used for chest pain, as well as recreational nitrites sometimes called “poppers.”

The labels also require caution with alpha blockers, some blood-pressure medicines, and drugs that change how the body processes PDE5 inhibitors. Riociguat, a soluble guanylate cyclase stimulator used for certain forms of pulmonary hypertension, is another contraindicated combination.
